该片改编自莫言1986年发表的两部中篇小说《红高粱》和《高粱酒》。因为一匹骡子，“ 我奶奶”九儿（巩俐饰）的父母要她嫁给一位拥有一座酒坊的50岁麻风病人。九儿乘坐花轿出嫁时，在途中被一名强盗拦截。其中一名抬轿男子余占鳌（姜文饰）打退了强盗，九儿因而和他开始相互喜欢。3天后九儿回乡探望父母 。抬轿男子突然从高粱田里冲出来，抢走了九儿。他们在高粱田野外交合。九儿回到酒坊的时候发现麻风病的丈夫死了，“ 我奶奶”撑起李家的烧酒作坊，不久余占鳌正式成为 “我爷爷”，并独创酿制好酒十八红的方法。抗日战争期间，“我爷爷”和“我奶奶”连手谱写了一曲悲歌。

本艳情片由李翰祥编导，四大艳星陈萍、余莎莉、陈思佳及邵音音领衔主演，另有姜戴维及李修贤客串。故事讲述，某酒厂职员兆实忆（刘一帆）死后经过鬼门关的时候，守关的小鬼说他死因不明，要他重新死过。当他重临世间时，却窥见妻子（余莎莉）与其好友吕品（田青）在偷欢；后来又听到儿子兆铭（华伦），跟媳妇商量拿得三万块保险金…… 故事分为三个，第一个说了一个倒霉蛋死后来到阴间，却因为死因不明被要求到阳间再死过，结果他回去后发现自己的老婆跟好友偷情，而儿子则等他的保险金来发展事业，他想找原来的老板借钱，结果把人吓到，最后想回去找个棺材还被其他鬼赶出……世态炎凉，一目尽现。 第二个故事则是一个书生借住鬼附近，遇到一怪人夜夜出没，奸污大屋里的女人。后来发现此人竟然是府中主人，所谓的QJ竟然是他的BT爱好……自己的还不如偷来的，老婆永远是别人的好。 第三个故事出自酒楼的怪谈，一酒坊的二当家因生意不佳，三当家不务正业，投缸而死，三当家却把泡过死人的酒卖出，生意还好的不得了，之前过世的大当家的儿子跟着两个叔叔混日子，三叔认为这个小侄子知道的太多，又要杀他灭口，岂料二当家鬼魂显灵，把恶毒的老三吓的失足摔入酒缸淹死……贪心不足，蛇肚子不够啊。 结果故事刚结束，故事里的两个主角就出现了，原来这个所谓的故事竟然是酒坊的竞争对手编出来打击对手的；一个如此可笑而意外的结局出现。这种峰回路转被后期很多西片运用，李导不愧是先锋！ 影片处处留有诙谐戏虐，演员发挥自然。剧本翔实，包袱互相衔接紧密，看完不觉其他小传类那种生变硬造的无稽，反而有点深省世间冷暖的意思；所以说拍SJ也可以拍的很艺术，很人文啊！
